Job description

Experienced Support Coordinator (Full-Time) for Essex, Hudson, Passaic Counties. (Bilingual Spanish Needed)
Also available Part-Time (Hudson, Passaic, Essex)
or  (Middlesex and Somerset)

We seek a self-motivated and compassionate individual with experience working with adults with intellectual and developmental disabilities.

Qualifications

  • One year of proven experience working with adults with developmental disabilities.
  • Candidates must be able to take the initiative, work independently, make executive decisions when necessary, and meet deadlines.
  • Excellent written and verbal communication, reliable car, flexibility to attend meetings when needed during the day.
  • This hybrid position requires field visits; candidates must be flexible to travel within these counties.
  • The state of New Jersey requires Covid vaccination for this position.
  • Must pass criminal background and CARI check.
  • A bachelor’s degree in psychology, sociology, social work, or related field.
Responsibilities

  • Assessing service needs, developing the New Jersey Individualized Service Plan (NJISP) for access to services, and liaison with clients, families, programs, and service providers.
  • To establish achievable goals, outcomes, and service eligibility.
  • Development of a Person-Centered Planning Tool (PCPT) is a mandatory discovery tool used to guide the person-centered planning process and assist in developing the NJISP.
  • Ongoing monthly monitoring of clients.
  • Coordinate and facilitate team meetings off-site at either Residential and/or Day Programs to identify service needs.
The caseload varies from 10 to 30 consumers.
Pay is $1,500 - $4,500 Monthly, commensurate with experience and caseload.
Must reside in the State of New Jersey
