Work Environment

This role primarily operates in a home care setting, allowing you to engage with patients in their comfortable environment. This model fosters a strong therapist-patient relationship and enables personalized care plans. The position is full-time, with a schedule of Monday through Friday from 2 PM to 8 PM, ensuring a predictable work-life balance.

Key Responsibilities

• Deliver high-quality, patient-centered care by managing the complete therapeutic process for an assigned caseload of patients. Coordination of care with both physicians and the patient care team is paramount.
• Carry out comprehensive patient assessments and actively participate in developing and implementing individualized care plans. Focus on achieving specific goals, enhancing patient engagement, and providing exceptional overall experiences.
• Conduct evaluations and apply treatment interventions within the scope of practice, adhering to established laws and professional codes of ethics.
• Assess and evaluate therapeutic, rehabilitative, and functional statuses, contributing to the formulation of a holistic total care plan.
• Deliver purposeful and skilled therapeutic care within patients’ homes, utilizing a variety of techniques to promote recovery or improvement in function in alignment with the physician’s care plan.
• Instruct patients, families, and caregivers on the appropriate use of therapies, exercises, and therapeutic appliances, ensuring effective implementation of care outside therapy sessions.
• Prioritize your caseload effectively, determining daily staffing needs, and coordinating assignments and hand-offs to optimize patient care and service delivery.

Patient Population / Client Focus

You will engage with a diverse patient population, including individuals recovering from surgeries, managing chronic conditions, or requiring rehabilitation following injuries. Your efforts will greatly impact their recovery trajectories, assisting them in regaining mobility, strength, and overall functionality.

Qualifications

• A current Physical Therapist license is required.
• A Doctor of Physical Therapy (DPT) degree is preferred.
• Basic Cardiac Life Support (BCLS) certification must be acquired within the first 30 days of employment.
• While a minimum of one year of experience is desired, motivated new graduates are encouraged to apply.

Preferred Experience

• Experience in home health settings is highly regarded but not mandatory.
• Familiarity with the diverse needs of pediatric and adult clients in physical therapy is beneficial.
