Full-time Hospice Nurse Case Manager
Location: Baker City, OR
Type: Direct Hire
Salary: $32 - $42/Hr
 
We are currently seeking a highly skilled and empathetic Full-time Hospice Nurse Case Manager to join our dedicated healthcare team in Baker City. This role is essential in delivering exceptional palliative care services, ensuring patients and their families receive the utmost support during their end-of-life journey. The ideal candidate will possess a profound understanding of hospice care, offering emotional guidance and comprehensive care planning to optimize patients' quality of life.
 
Key Responsibilities:
  • Conduct thorough assessments to develop personalized care plans for patients.
  • Coordinate and manage patient visits, ensuring alignment with their needs and physician directives.
  • Manage medication administration and orders in a timely manner.
  • Supervise and support LPNs, LVNs, and Hospice Aides as per established protocols.
  • Collaborate with an interdisciplinary team to provide holistic care, addressing physical, emotional, and spiritual needs.
  • Educate families and caregivers, providing resources to support them during this challenging time.
  • Maintain a compassionate and respectful approach, upholding the values of empathy and integrity in all interactions.
Qualifications:
  • Associate's degree preferred; Registered Nurse license required.
  • At least one year of nursing experience, with prior hospice experience favored.
  • Willingness to participate in on-call rotations.
  • Possession of reliable transportation and a valid driver's license.
