Position Overview

Life Stage Counseling is seeking a dedicated Licensed Master Social Worker (LMSW) to join our compassionate team, focusing on providing high-quality support to seniors in Fort Collins, Colorado. Key Responsibilities
  • Conduct thorough psychosocial assessments to identify the emotional, mental, and social needs of seniors.
  • Develop and implement personalized care plans in collaboration with clients and their families, ensuring individualized approaches to care.
  • Provide counseling services and emotional support, helping seniors navigate significant life transitions, including health changes or loss.
  • Advocate for clients, facilitating access to essential community resources and services that support independent living.
  • Work in partnership with an interdisciplinary team, which includes healthcare professionals like nurses and caregivers, to deliver coordinated care.
  • Perform regular client visits to monitor well-being, assess living conditions, and address potential safety or health concerns.
  • Assist families in communication and conflict resolution regarding their loved ones' care, ensuring all voices and concerns are heard.
  • Educate clients and their families on community programs that promote social engagement, wellness, and overall quality of life.
  • Maintain accurate and timely documentation of clients' assessments, care plans, interactions, and progress to ensure continuity of care.
  • Stay informed about best practices in social work, ethical standards, and regulatory requirements to provide compliant and effective services.
Patient Population / Client Focus

Your primary focus will be seniors within the Fort Collins community, many of whom may be grappling with issues such as isolation, coping with chronic illnesses, or transitioning to new living situations. It’s imperative to understand the diverse backgrounds and experiences of these clients to tailor approaches that respect their individual histories and needs. You will be a key figure in helping them achieve their personal goals and enhance their overall quality of life.

Qualifications
  • Master's degree in Social Work from an accredited institution.
  • Current, valid state licensure as a Licensed Master Social Worker (LMSW).
  • At least two years of experience in social work, preferably within a community health or geriatric environment.
  • Strong understanding of aging issues, social work principles, and available community resources for seniors.
  • Exceptional assessment and problem-solving skills, with the ability to develop meaningful care plans.
  • Empathetic nature with a passion for supporting seniors and their families, establishing strong rapport and trust.

  • Proficiency in electronic health records (EHR) systems and other relevant technology.
Preferred Experience

Compensation and Benefits

The compensation for this role ranges between $60,000 to $100,000 annually, commensurate with experience and qualifications.
