Work Environment

This position is designed to be remote, allowing for flexibility and the ability to serve clients from various locations within Denver. As a member of our supportive team, you will engage in telehealth services, conduct virtual assessments, and maintain regular communication with clients and their families. The work schedule generally runs from Monday to Friday, 8:00 AM to 4:30 PM, although flexibility is essential to meet clients' needs, which may include occasional evening or weekend hours.

Key Responsibilities
  • Conduct comprehensive psychosocial assessments of seniors to identify emotional, mental, and social factors influencing their overall well-being.
  • Collaborate closely with clients and their families to develop and implement personalized care plans tailored to individual goals.
  • Provide ongoing counseling and emotional support, assisting clients in managing life transitions, health changes, and caregiving challenges.
  • Serve as an advocate for clients’ rights, guiding them through available community resources and services needed for optimal care.
  • Work within interdisciplinary teams, connecting with healthcare providers, caregivers, and support staff to coordinate effective and holistic care.
  • Regularly monitor clients' health and living conditions through virtual check-ins and home visits as necessary.
  • Educate families on available resources, workshops, and support groups to enhance engagement and prevent isolation.
  • Manage and document all client interactions meticulously, maintaining up-to-date records in compliance with agency standards and ethical guidelines.
  • Stay current on best practices and regulations in social work, particularly those relevant to aging populations, ensuring high standards of care.
Patient Population / Client Focus

This position primarily focuses on seniors facing various challenges associated with aging, including mental health issues, chronic illness management, and the emotional complexities related to transitioning into different stages of life. The Social Worker will engage with clients, their families, and caregivers to foster an environment of understanding, empathy, and effective communication.

Qualifications
  • Master's degree in Social Work (MSW) from an accredited institution.
  • Active and valid state licensure as a Licensed Master Social Worker (LMSW).
  • A minimum of two years of relevant social work experience, ideally focused on geriatric populations or in community settings.
  • Strong understanding of social work practices, theories, issues related to aging, and community resources available for seniors.
  • Excellent communication, assessment, and problem-solving skills.
  • Demonstrated ability to build rapport and establish trust with seniors and their families effectively.
  • Proficiency in utilizing Electronic Health Record (EHR) systems and telehealth technologies.
Preferred Experience
  • Experience in a community center or healthcare organization specializing in senior services.
  • Familiarity with care coordination among interdisciplinary teams.
  • Knowledge of health and wellness programs targeting older adults.

Life Stage Counseling offers a competitive salary range of $60,000 to $100,000 per year, commensurate with experience and qualifications.
