Key Responsibilities
  • Conduct intensive counseling and psychotherapy sessions, offering tailored treatment services to clients in need.
  • Perform comprehensive mental status evaluations to assess clients' psychological conditions and recognize any changes over time.
  • Document client progress through clear and thorough case notes and treatment plans, ensuring compliance with organizational standards.
  • De-escalate high-stress situations with a calm and empathetic approach, enabling the client to feel heard and understood.
  • Collaborate with psychiatrists, healthcare professionals, and community resources to provide well-rounded care and support.
  • Facilitate therapeutic interventions in both individual and group settings, adapting techniques to the specific needs of clients.
  • Engage in team meetings and training sessions, sharing insights and strategies to improve overall patient care.
Patient Population / Client Focus

This role involves providing support to a diverse range of clients, predominantly within the hospice care context. You will work closely with patients facing terminal illnesses and their families, helping them navigate emotional, psychological, and social challenges. Understanding the unique needs of individuals in end-of-life situations is crucial as you provide compassionate support during a critical period of their lives.

Qualifications
  • A Master’s degree in Social Work from an accredited institution.
  • A valid Pennsylvania LCSW license to practice independently.
  • Exceptional interpersonal skills with the ability to establish rapport and build trust with clients of diverse backgrounds.
  • Strong understanding of therapeutic modalities and experience in delivering individual and group therapy.
  • Excellent organizational skills with the ability to manage multiple cases effectively.
  • Proficient verbal and written communication skills for clear and effective documentation and interaction with clients and team members.
Preferred Experience
  • At least two years of experience in a clinical social work setting, particularly in hospice, hospital, or community mental health environments.
  • Experience with trauma-informed care and grief counseling is highly desirable.
  • Familiarity with electronic health record (EHR) systems to document client interactions and care plans.
