Overview
  • We seeking to partner with a dedicated and compassionate Nurse Practitioner to deliver advanced in-home wound care for chronic and complex wounds. The ideal candidate will provide high-quality patient care, perform assessments, and develop treatment plans in the patient's home or potentially in your office. This role requires a strong commitment to patient advocacy and the ability to work collaboratively with other healthcare professionals.

Duties
  • Conduct comprehensive wound examinations and health assessments for patients upon referral. Debride and graft chronic wounds that have not healed with at least 4 weeks of conservative treatment. Collaborate with physicians and other healthcare providers to ensure coordinated care. Maintain accurate medical records and documentation in compliance with healthcare regulations.

Skills
  • Independent Medicare Part B credentials are strongly preferred. Experience in wound care is highly desirable. Ability to manage medical records efficiently while ensuring patient confidentiality. Excellent communication skills for effective patient interaction and collaboration with healthcare teams. Strong organizational skills to manage multiple patient cases simultaneously.

  • Already Credentialed with Medicare & Advantage Plans
  • LLC already created
  • Ability to work mobile (Treat Patient in-Home)
  • Wound Care and Advanced Wound Care experience

Qualifications:
  • Certified Nurse Practitioner
  • M.D's (Can qualify for this job)
  • P.A.'s (Can quality for this job)
