Work Environment

This position offers a hybrid work environment, allowing you to work remotely while also connecting with clients in person at our dedicated Hospice Center in Modesto, CA. Key Responsibilities
  • Conduct one-on-one counseling sessions via secure video chat and in-person appointments to support clients' mental health needs.
  • Utilize clinical expertise in assessing clients’ mental, emotional, and social needs, ensuring that treatment approaches are based on research-backed methodologies.
  • Develop personalized treatment plans that respond to the unique needs and goals of each client, navigating challenges such as grief, anxiety, and depression within hospice settings.
  • Facilitate regular case reviews and supervision sessions with the broader treatment team, collaborating closely with physicians, nurses, and other therapists to enhance the quality of care provided.
  • Advocate for clients on various fronts, helping them establish realistic and measurable goals for personal growth while navigating the complexities of end-of-life care.
  • Maintain thorough and accurate documentation of client progress, treatment plans, and other relevant information in compliance with organizational policies and state regulations.
Patient Population / Client Focus

In this role, you will specifically work with clients and families in the hospice care environment. This involves addressing the emotional and psychological challenges faced by patients nearing the end of life, their family members, and caregivers. Your expertise will contribute significantly to enhancing their quality of life, providing them with coping strategies, and facilitating healthy communication during vulnerable times.

Qualifications
  • A Master's degree in Social Work (MSW) from an accredited program.
  • Current and valid California state license as a Licensed Clinical Social Worker (LCSW) with no disciplinary actions.
  • Proven experience in clinical social work, particularly in the areas of assessment, diagnosis, and treatment planning, ideally within a healthcare setting.
  • Exceptional communication and interpersonal skills, enabling you to build trust and rapport with a diverse clientele.
  • Strong technological proficiency, with the ability to use various digital tools for teletherapy and remote patient management effectively.
Preferred Experience
  • Experience working in hospice or palliative care settings, particularly with terminally ill patients and their families.
  • Background in grief counseling, trauma-informed care, or crisis intervention strategies.
  • Familiarity with the social determinants of health and how they impact mental health outcomes.
Compensation and Benefits

We offer a competitive annual salary ranging from USD 115,000 to 130,000, commensurate with experience and qualifications.
