Key Responsibilities
  • Conduct thorough psychosocial assessments to collect detailed information on clients' mental health concerns and existing resources.
  • Facilitate individual, family, and group therapy sessions employing a variety of evidence-based treatment modalities.
  • Utilize specialized therapies such as cognitive-behavioral therapy (CBT), solution-focused therapy, and trauma-informed care tailored to individual client needs.
  • Design and execute personalized treatment plans with clearly defined goals and therapeutic interventions.
  • Collaborate with a multidisciplinary team to ensure that treatment plans are client-centered and holistic.
  • Provide crisis intervention services, including risk assessments and safety planning for clients in acute distress.
  • Maintain accurate and timely documentation in compliance with all ethical, legal, and professional standards.
  • Lead group therapy sessions and psychoeducation workshops to facilitate knowledge sharing among clients.
Patient Population / Client Focus

Your work will focus on a unique patient population within the hospice care setting. You will support individuals dealing with terminal illnesses and assist their families in navigating the emotional complexities of the end-of-life process. This position allows you to make a profound impact on the quality of life and emotional support provided to both clients and their families during such critical moments.

Qualifications
  • An MSW (Master of Social Work) degree from an accredited institution.
  • Active state licensure as a Licensed Clinical Social Worker (LCSW) to practice in New Jersey.
  • A minimum of two years of experience working in a mental health or outpatient setting, preferably in hospice or palliative care.
  • Demonstrated knowledge of evidence-based treatment modalities and therapeutic approaches.
  • Exceptional interpersonal and communication skills, with a strong ability to empathize and engage with clients from diverse backgrounds.
Preferred Experience
  • Experience providing care in hospice or palliative settings is highly desirable.
  • Knowledge of community resources and referral processes to assist clients in accessing supplementary support services.
  • Familiarity with electronic health record (EHR) systems for managing clinical documentation efficiently.
Compensation and Benefits
