Work Environment

The position is primarily onsite, with physical therapy services rendered directly in patients' homes across the Cresskill region and surrounding areas. This home care model not only offers unique challenges but also allows for a personalized touch in rehabilitative practices. You will work Monday to Friday from 9 AM to 5 PM, focusing exclusively on patient care without the distractions of a clinic environment.

Key Responsibilities
  • Conduct comprehensive physical assessments to identify movement disorders, strength deficits, balance issues, and mobility restrictions of each patient.
  • Utilize evidence-based methodologies to determine the root causes of pain and dysfunction, allowing for accurate diagnosis and effective treatment.
  • Develop and implement individualized rehabilitation programs tailored precisely to each patient's needs, goals, and progress tracking.
  • Provide hands-on treatment integrating therapeutic exercises, manual therapy techniques, and injury prevention strategies.
  • Maintain thorough, timely documentation of patient care, progress, and treatment outcomes using our electronic health record (EHR) system.
  • Educate patients and their families on injury prevention techniques, rehabilitation exercises, and overall wellness strategies to promote long-term health.
  • Participate in regular team meetings to discuss patient progress and engage in continuous professional development opportunities.
Patient Population / Client Focus

This role primarily involves working with a diverse patient population, including elderly individuals recovering from surgeries, athletes rehabilitating from injuries, and adults managing chronic pain conditions. The position demands a sensitive, patient-centered approach, emphasizing the need for compassion and understanding in order to foster trust and cooperation.

Qualifications
  • A current and valid Physical Therapist (PT) license in the state of New Jersey.
  • A Doctor of Physical Therapy (DPT) degree or its equivalent.
  • Two years of clinical experience in a physical therapy setting, preferably with an emphasis on home care or outpatient services.
  • Exceptional communication and interpersonal skills, allowing for effective interaction with patients, families, and colleagues.
  • A proven ability to develop and implement effective patient-specific treatment plans based on thorough assessments.
  • Demonstrated commitment to a compassionate, patient-centered approach to care.
  • Ability to work both independently and collaboratively within a multidisciplinary team.
Preferred Experience
  • Experience in a home health care setting, with an understanding of the unique challenges and rewards that come with providing therapy in a patient's residence.
  • Familiarity with electronic health records and a commitment to maintaining accurate, up-to-date patient documentation.
  • Experience working with varied patient demographics, focusing on orthopedic and rehabilitative care.
