Job description

Homeward Bound, a behavioral health agency for adults, is hiring a Qualified Mental Health Professional to work on the Deflection/Crisis Respite Unit where residents may stay for 1-10 days while they get stabilized due to being off of medication, experiencing housing instability, or needing coordination with other types of care but without the acuity of needing a locked down facility.

Candidates must have a minimum of a bachelor's level education in psychology, counseling, sociology, social work, or related field.

Job duties will include

  • Complete an initial screening and/or assessment on each individual within 24 hours of arrival on the crisis unit.
  • Assist residents with all case management needs including, but not limited to, appropriate housing upon discharge, connection with positive social support, and coordination with necessary resources.
  • Coordinate care between deflection/crisis respite and mental health outpatient and other aftercare providers.
  • Complete intake documentation with clients including financial information.
  • Prepare residents in deflection/crisis respite for discharge and work with the treatment team to eliminate barriers to success.
  • Create person-centered treatment plans with individualized goals, objectives, and strategies.
  • Assist in direct care on the unit including observation, checking in and out belongings, facilitating phone calls, and attending meals.
  • Facilitate psychoeducational groups daily and complete documentation.
  • Provide therapeutic intervention in potential crisis situations and assist the team in ensuring the appropriate level of care and coordination of services is provided.
  • Attend all treatment team staff meetings.
  • Perform a daily evaluation and documentation of each client’s mental status including assessing for suicidal ideation, homicidal ideation, and audio/visual hallucinations.
  • Be willing to take and document vitals for clients as needed.
  • Be trained in self-administration of medication and give medication to clients when needed.
  • Assist in clinical decision making regarding both admissions and discharges.
  • Be responsible for scheduling and maintaining communication with both residential and outpatient clients.

  • Be familiar with different funding sources and the documentation that each requires.
