Job Title: Registered Nurse (RN) Case Manager - Home HealthBuild Meaningful Patient Relationships
Company: Home Health Care, Inc.
Location: Burnsville and Coon Rapids + 35 mile radius from your home address
Schedule: Full-TimeMonday through Friday, 8:00 AM to 5:00 PM, 1 weekend per month, 1 on-call shift per week
Compensation: $85,000 to $130,000 annually plus $6,000 in first-year bonuses

As a Case Manager, you're more than a visiting nurse.
You're the clinician responsible for guiding each patient's care from start to finish.
You'll coordinate services, communicate with providers, educate families, monitor progress, and adjust care plans as patient needs evolve.

What You'll Be Doing
  • Manage an assigned home health patient caseload
  • Complete OASIS assessments and individualized care plans
  • Perform skilled nursing visits, assessments, and patient education
  • Coordinate care with physicians, therapists, social workers, and interdisciplinary team members
  • Monitor patient progress and identify changes in condition
  • Educate patients and caregivers on medications, disease management, and recovery
  • Complete timely documentation using Homecare Homebase (HCHB)
  • Ensure patients receive coordinated, high-quality care throughout their episode

Who We're Looking For
  • Active Minnesota Registered Nurse (RN) license
  • Three or more years of RN experience
  • Home health experience preferred
  • Strong clinical assessment and critical thinking skills
  • Comfortable working independently while managing a caseload
  • Excellent communication and organizational abilities
  • Reliable transportation for local travel
