Job description

Care Coordinator

Location: Columbus, OH

Job Type: Full-time

Griswold Home Care of Franklin/Delaware counties is a well-established, respected provider of non-medical home care services. We are currently seeking a Care Coordinator to join our team. The successful candidate will be responsible for coordinating and managing all aspects of home care services for our clients.

Responsibilities
  • Manage and coordinate the scheduling of a caseload
  • Evaluate clients' needs both via the phone and in person at client's home, facility or hospital to match the right caregiver with the client, both in skill level and personality
  • On boarding of new clients and caregivers.
  • Visit clients to ensure satisfaction
  • Focus on the quality of care and build warm client, family, and caregiver relationships
  • Provide guidance and loving support to clients, families, and caregivers to ensure an exceptional home care experience
  • Mandatory after hours "on call" rotation. (extra $550/week when on call)
Requirements
  • Caring, empathetic, and compassionate individual
  • Effective communicator with a pleasant phone manner.
  • Strong verbal & written skills
  • Ability to build strong rapport with caregivers, clients and families
  • Creative problem-solving skills
  • Organized, detail-oriented multitasker
  • Good documentation, clerical and follow up skills
  • Computer proficiency
  • Experience with WellSky software is a plus.
