Work Environment

This position is based onsite at our rehabilitation facility, which is equipped with state-of-the-art rehabilitation technology and resources. Key Responsibilities

• Conduct evaluations of patients, reviewing their medical history, performing necessary assessments, and formulating comprehensive treatment plans tailored to individual needs.
• Communicate evaluation findings effectively to patients and their families, involving them in the goal-setting process to ensure personalized care.
• Implement treatment protocols with proficiency, adapting techniques based on evaluation outcomes and the dynamic needs of the patient.
• Collaborate with a multidisciplinary team of healthcare professionals, recognizing and documenting changes in patient conditions that necessitate plan modifications or additional consultations.
• Manage discharge planning and follow-up care, ensuring that all documentation accurately reflects the patient’s treatment journey, including detailed notes on evaluations, progress, and discharge summaries.
• Demonstrate effective time management skills by meeting productivity expectations and maintaining an organized schedule.
• Contribute to departmental goals by participating in community outreach programs and marketing activities, promoting therapy services and improving community access to care.
• Engage in ongoing personal and professional development through continuing education opportunities, enhancing your competencies and contributing to team growth.
• Actively participate in removing barriers to patient access and understanding site performance metrics to improve service delivery.

Qualifications

• Bachelor's Degree in Physical Therapy (DPT) or an equivalent qualification.

• A minimum of 2 years of experience in a clinical or rehabilitation setting is preferred, though recent graduates will be considered.
• Experience in outpatient settings is a plus, with familiarity in evidence-based treatment methods and protocols.
