RN Registered Nurse Home Health Clinical Director

Job Summary:

We are seeking a highly motivated and experienced RN Registered Nurse Home Health Clinical Director to join our team at Deer Meadows Home Health in Philadelphia, Pennsylvania. As a RN Registered Nurse Home Health Clinical Director, you will be responsible for overseeing the clinical operations of our home health department, ensuring high-quality patient care and compliance with regulatory requirements.

About the Home Health Clinical Director Role:

As a RN Registered Nurse Home Health Clinical Director, you will be responsible for:

  • Leading and directing a team of home health the agency clinical leadership (RN Clinical Manager & DOR), ensuring effective communication and collaboration to achieve departmental goals and objectives

  • Providing clinical guidance and mentorship to staff members, promoting evidence-based practice and continuous quality improvement

  • Developing and implementing patient care plans and programs, ensuring compliance with regulatory requirements and accreditation standards

  • Facilitating interprofessional collaborations with other departments, including rehabilitation, therapy, and social work services

  • Analyzing clinical data and metrics to identify trends and opportunities for improvement

Home Health Clinical Director Responsibilities:

  • Supervise and coordinate the activities of the home health department, ensuring efficient and effective use of resources

  • Develop and maintain relationships with referring physicians, patients, and families to ensure high-quality patient care and satisfaction

  • Collaborate with other departments to develop and implement education and training programs for staff members

  • Conduct ongoing clinical evaluations and assessments of patient care, identifying areas for improvement and implementing corrective actions

  • Maintain accurate and up-to-date records of patient care, including electronic medical records and departmental data

Home Health Clinical Director Requirements:

  • Current RN license in the state of Pennsylvania

  • Minimum 2-3 years of experience in home health or related field, with at least 1-2 years in a leadership or management role

  • Strong clinical knowledge and experience in home health care, with ability to adapt to changing regulations and standards

  • Excellent communication, leadership, and organizational skills, with ability to work effectively in a fast-paced environment
