Job description

Physical Therapist (PT) – Full-Time

Deer Meadows Rehabilitation Center – Philadelphia, PA

Deer Meadows Rehabilitation Center is seeking a Full-Time Physical Therapist (PT) to join our in-house rehabilitation team. This is an excellent opportunity for a motivated therapist who is passionate about helping patients maximize their independence and quality of life in a skilled nursing and long-term care setting.

Responsibilities

  • Evaluate residents and develop individualized treatment plans

  • Provide skilled physical therapy services to SNF/LTC residents

  • Establish functional goals and monitor patient progress

  • Collaborate with physicians, nursing staff, therapy team members, residents, and families

  • Complete timely and accurate documentation utilizing Net Health and PointClickCare (PCC)

  • Participate in care planning and interdisciplinary team meetings

  • Ensure compliance with all federal, state, and facility regulations

Qualifications

  • Graduate of an accredited Physical Therapy program

  • Current Pennsylvania PT license or eligibility for licensure

  • Strong communication and interpersonal skills

  • Ability to work independently and as part of a team

  • Experience in SNF/LTC preferred, but not required

  • New graduates welcome to apply
