Key Responsibilities
  • Conduct comprehensive assessments of patients to identify their mental health needs, ensuring tailored care plans are developed.
  • Maintain open lines of communication with supervisors, direct care staff, families, and other stakeholders regarding patient progress and treatment plans.
  • Organize and provide a detailed schedule for appointments, home visits, and assessments, to ensure thorough monitoring of patient treatment.
  • Conduct home visits as necessary, involving observations and data collection to support ongoing care requirements.
  • Ensure the effective implementation of treatment plans and provide regular updates during Interdisciplinary Team Meetings.
  • Document progress monthly and quarterly to track treatment outcomes and patient advancements.
  • Develop and organize training programs for staff to improve their competencies in program implementation and documentation.
  • Provide ongoing support through follow-up interviews and observations, fostering the continued development of patients’ wellbeing.
  • Act as a liaison within the broader mental health community, assisting in the collaboration toward collective development goals.
  • Contribute to the creation in the establishment of procedures and training materials designed to enhance program effectiveness.
Patient Population / Client Focus

Your role will involve working with a diverse population that includes both adults and youth. As a mental health counselor, you will provide support and guidance tailored to the unique needs of each individual, focusing on various challenges such as anxiety, depression, trauma, and family-related issues. Your approach will prioritize understanding and empathy, fostering a therapeutic environment conducive to healing.

Qualifications
  • A Master’s or Doctoral degree in Counseling, Psychology, or a related field is required.
  • Possession of an active and unrestricted license as a Professional Counselor (LPC) in Arizona.
  • A minimum of two years of professional counseling experience is mandated, with a preference for clinical experience within a community health setting.
  • Demonstrated specialization in areas such as family therapy, couples therapy, substance misuse, trauma, or similar fields is highly preferred.
  • Comprehensive understanding of legal and ethical standards governing mental health practice.
  • Exceptional interpersonal and communication skills to establish rapport with a wide range of clients.
  • Ability to collaborate effectively within an interdisciplinary healthcare team.
  • Familiarity with EMR software and related technologies essential for patient management and documentation.
Preferred Experience
  • Experience working in community health clinics or similar environments.
  • Knowledge of evidence-based treatment methodologies applicable to specified patient populations.
  • Previous experience in conducting training and development workshops for mental health staff.
Compensation and Benefits
