Key Responsibilities
  • Conduct thorough psychosocial assessments that encompass emotional, social, and financial aspects impacting patient well-being.
  • Develop individualized treatment plans in collaboration with medical professionals, ensuring comprehensive care that addresses both behavioral health and physical health needs.
  • Provide both individual and group counseling, focusing on themes such as grief, coping strategies, and adjustment to treatment.
  • Coordinate discharge plans and facilitate access to community resources post-treatment, ensuring continuity of care and support.
  • Engage with community organizations to implement post-discharge services tailored to the unique needs of each patient.
  • Offer education and emotional support to patients and families, helping them navigate treatment options and resources effectively.
  • Document all assessments, treatment plans, and interventions in accordance with legal and ethical standards, maintaining the confidentiality of patient information.
  • Collaborate with hospital committees to contribute to policy development and best practices in social work.
  • Provide crisis intervention and support during high-risk situations to ensure patient safety and well-being.
Patient Population / Client Focus

Qualifications
  • Master's degree in Social Work from an accredited institution.

  • Strong ability to communicate effectively with patients, families, and interdisciplinary team members.
Preferred Experience

Compensation and Benefits
