  • Maintain trusting relationships with clients and families, demonstrating respect for their diverse backgrounds, beliefs, and needs.
  • Ensure adherence to professional ethical guidelines and the conduct code established by Community Outreach Network, emphasizing the importance of integrity and confidentiality.
  • Identify clients’ immediate and ongoing needs through effective screening processes and arrange for thorough assessments as required.
  • Create and implement integrated care plans, ensuring continuity of care through coordination with external services and support systems.
  • Provide focused prevention, treatment, and recovery services tailored to individual client goals.
  • Engage in continuous professional development, monitoring your own performance through feedback and accessing further training opportunities.
  • Participate in committees focused on improving service delivery and quality within the agency.
Patient Population / Client Focus

This role will involve working with a diverse clientele, including individuals at the end-of-life stage, their families, and other vulnerable populations requiring emotional and mental health support. The focus will be on understanding the unique challenges faced by those dealing with terminal illness and providing compassionate care that empowers clients and their families.

Qualifications
  • A Master’s Degree in Social Work from an accredited institution is required.
  • Must be a Licensed Independent Clinical Social Worker (LICSW) in the state of Washington.
  • Demonstrable expertise in assessment, diagnosis, and therapy for individuals with mental health challenges.
  • Strong verbal and written communication skills to effectively engage with clients and colleagues.
  • Exceptional organizational and time management skills, with the ability to manage a caseload effectively.
  • Competency in addressing age-related and specialty issues pertinent to our diverse client population.
Preferred Experience

Candidates should have a minimum of two years of relevant clinical experience, preferably within hospice or community health settings. Experience in crisis intervention strategies and familiarity with integrated care models will be advantageous. Knowledge of community resources in Spokane and strong networking capabilities will also enhance your effectiveness in this role.

Compensation and Benefits

The position offers a competitive annual salary ranging from $70,000 to $85,000, commensurate with experience. Additional benefits include health insurance, paid time off, professional development stipends, and retirement savings options to support your long-term financial well-being.
