Key Responsibilities

• Conduct thorough assessments of clients to identify their social, emotional, and environmental challenges, utilizing standardized assessment tools and interpersonal techniques.
• Develop and implement individualized service plans that include defined goals, intervention strategies, and continuous progress monitoring to ensure measurable outcomes for clients.
• Provide both individual and family counseling sessions, guiding clients through their life challenges with empathy and evidence-based therapeutic methodologies.
• Collaborate closely with clients to establish personalized action plans and connect them to essential community resources, ensuring they receive holistic support.
• Administer crisis intervention services, performing risk assessments and safety planning for at-risk individuals and families in immediate need.
• Engage actively with community agencies and organizations to coordinate services and advocate on behalf of clients, helping to remove barriers to care.
• Participate in outreach initiatives to educate the community about available services, contributing to awareness and organizational visibility.
• Take part in case conferences, cross-disciplinary team meetings, and ongoing professional development training, enhancing both individual and team capabilities.

Qualifications

Compensation and Benefits
