Community Bridges is seeking Direct Support Professionals (DSPs) to join our Day Program in Franklin, NH.
Founded in 1982, Community Bridges is a nonprofit organization dedicated to advancing the integration, growth, and interdependence of people with developmental disabilities within their communities. We support more than 1,100 individuals and families throughout the Greater Capital Area by identifying barriers, developing creative solutions, and fostering meaningful community engagement.
Position Overview
As a Day Program Direct Support Professional, you will promote abilities-not disabilities by supporting individuals in building skills, relationships, and community connections. You will play a key role in helping individuals identify and build upon their unique strengths, interests, and gifts to support meaningful social inclusion.

Key Responsibilities
Support individuals in building confidence, self-esteem, and independence through planned activities and community involvement
Provide individualized supports, including personal care and skill development
Assist individuals in presenting a positive self-image
Participate in Person-Centered Planning and implementation of goals
Complete required documentation, including data collection and monthly progress notes
Maintain compliance with required state and agency trainings (Relias and in-person)
Report safety, health, or human rights concerns immediately
Follow state regulations and agency policies
Perform additional duties as assigned

Work Environment & Physical Demands
Travel Requirements
Access to a personal vehicle during all scheduled work hours
Frequent travel with individuals supported
Valid NH driver's license and safe driving record
