Job description

COMMUNITY BRIDGES IS GROWING - JOIN OUR TEAM! 

Community Bridges is seeking Direct Support Professionals (DSPs) to join our Day Program in Franklin, NH.

Founded in 1982, Community Bridges is a nonprofit organization dedicated to advancing the integration, growth, and interdependence of people with developmental disabilities within their communities. We support more than 1,100 individuals and families throughout the Greater Capital Area by identifying barriers, developing creative solutions, and fostering meaningful community engagement.


Position Overview

As a Day Program Direct Support Professional, you will promote abilities-not disabilities by supporting individuals in building skills, relationships, and community connections. You will play a key role in helping individuals identify and build upon their unique strengths, interests, and gifts to support meaningful social inclusion.


Qualifications

Education:

  • High School Diploma or GED required

Experience:

  • Experience in human services or working with individuals with disabilities is preferred

Additional Requirements:

  • Reliable transportation

  • Valid driver's license

  • Proof of automobile insurance


Compensation & Benefits
  • Paid Time Off (PTO):

    • 22 days annually for full-time employees (40 hours/week)

Current Open Positions
  • Part-time: 15 hours a week Franklin, NH (Day Program)

Part-time positions may be combined to create full-time hours.


Key Responsibilities
  • Support individuals in building confidence, self-esteem, and independence through planned activities and community involvement

  • Provide individualized supports, including personal care and skill development

  • Assist individuals in presenting a positive self-image

  • Participate in Person-Centered Planning and implementation of goals

  • Complete required documentation, including data collection and monthly progress notes

  • Maintain compliance with required state and agency trainings (Relias and in-person)

  • Report safety, health, or human rights concerns immediately

  • Follow state regulations and agency policies

  • Perform additional duties as assigned

  • Ability to meet documentation deadlines

  • Ability to work independently and exercise sound judgment

  • Ability to support daily living and functional skills

  • Ability to utilize community resources and opportunities

  • Team-oriented with the ability to follow team decisions

  • Ability to remain calm and positive in stressful situations


Work Environment & Physical Demands
  • Environment: Primarily community-based; office attendance required for trainings and meetings

  • Physical Demands:

    • Ability to lift up to 50 lbs

    • Frequent walking and standing

    • Occasional stair climbing

    • Ability to push wheelchairs and operate lifts as needed

    • Ability to follow behavioral intervention techniques


Travel Requirements
  • Access to a personal vehicle during all scheduled work hours

  • Frequent travel with individuals supported

  • Valid NH driver's license and safe driving record

  • Adequate automobile liability insurance
