    POSITION SUMMARY: The Bloom Beyond Program Coordinator plays a vital role in supporting transition-aged youth (18-26) who have experienced foster care through personalized navigation support, life skills development, and program coordination. This position will focus on helping young adults access community resources, develop independent living skills, and build strong support networks while ensuring program quality and impact.
    PRIMARY RESPONSIBILITIES:
    • Navigation Services & Case Management
    • Provide one-on-one support to young people in accessing resources and achieving personal goals
    • Develop and maintain relationships with community partners and service providers
    • Coordinate resource referrals and follow-up to ensure successful connections
    • Maintain detailed case notes and documentation of participant interactions
    • Help participants organize important documents and complete necessary applications
    • Track participant progress and outcomes
    • Leadership Council Coordination
    • Help to lead recruitment, engagement, and retention of council members
    • Help to plan and facilitate monthly council meetings
    • Implement council initiatives and recommendations
    • Help to manage participation tracking
    • Help create meaningful leadership development opportunities for members
    • Foster peer connections and support among council participants
    • Program Development & Management
    • Contribute to curriculum development for independent living skills training
    • Facilitate workshops and educational sessions for young people
    • Coordinate with Bloom Closet team to ensure participant access
    • Assist in program evaluation and continuous improvement efforts
    • Track and report program metrics and outcomes
    • Develop and maintain program materials and resources
    • Support outreach and recruitment efforts
    • Administrative & Team Support
    • Maintain accurate and up-to-date participant records
    • Contribute to grant reports and program documentation
    • Participate in team meetings and professional development
    • Assist with special projects and events as needed
    • Support intern supervision and training as assigned • Other duties as assigned.
    REQUIREMENTS:
    • Bachelor of Social Work or related profession (master's degree preferred) or lived experience in foster care
    • At least 2-3 years of experience working with youth/young adults, preferably those with foster care experience
    • Strong case management and resource navigation skills
    • Demonstrated experience in program coordination and implementation
    • Excellence in relationship building and communication across diverse populations
    • Proficiency in data collection, documentation, and reporting
    • Knowledge of trauma-informed care principles and practices
    • Understanding of metro Atlanta social services and community resources
    • Valid driver's license and reliable transportation
