Description:

JOB SUMMARY: Under the direction of the VP of Home and Care Services and the

Director of Enhanced Care Management, Administrative Assistant II supports Enhanced

Care Management (ECM) services by overseeing client referral flow and maintaining daily

operational records. This position requires knowledge and experience in medical office

operations and business office management. Strong skills in accurate data entry,

attention to detail, and customer service are essential.

This position requires proficiency in word processing, Microsoft Excel and database programs

with an emphasis on accuracy and speed.

ESSENTIAL DUTIES AND RESPONSIBILITIES:

1. Provides administrative and clerical support the Enhanced Care Management, Care

Coordination programs, and other duties arise as the program continues to grow capacity

2. Screens and logs incoming patient referrals, distributes to the relevant department.

Manages general agency phone calls and provides information to callers, and routing calls

to appropriate staff members using a multi-line console.

3. Make use of relevant automated office management systems, Electronic Medical records

for effective functioning of the programs and kept updated

4. Processing Enhanced Care Management, Care Coordination programs patient eligibility,

and others as directed by Management

5. Conduct member outreach via phone and text, and Pear Suite Call Center

documentation

6. Document outreach and engagement in CalOptima Connect and Pear Suite

7. Schedule assessments with new members based on Care Coordinator

availability

8. Update trackers, maintain accurate records, and support ECM leadership

reporting

9. Take meeting minutes for contractor meetings

10. Responds to incoming patient calls, program related communication, emails and meetings

notes

11. Oversees inventory and ordering office supplies, and medical supplies

12. Operates and maintains various office equipment, including, but not limited to: copier, fax

machine and postage meter.

13. Performs other duties and/or office support as required by supervisors relevant to the

successful operation

Requirements

EDUCATION, EXPERIENCE AND QUALIFICATIONS:

• High School Diploma and 2-3 years’ experience as an Administrative Assistant/Associate

or Executive Assistant. Preferred in Health Care environment

• Experience in case management, care coordination, or community health +

• Bilingual (Spanish preferred) +

• Working knowledge of computerized health systems, insurance payors and processing

or other related office functions.

• Strong interpersonal skills, including the ability to interact well with clients, visitors and

staff.
