Schedule: Full-Time | 36 - 40 hours per week
Compensation: $20.00 - $26.50 per hour; based on experience and licensing

The ideal candidate builds strong relationships, communicates effectively, and works collaboratively to promote whole-person care.
What We Offer- Status: Full-Time, 36 - 40 hours per week
- Schedule: Monday - Friday
- Certified Nursing Assistant (CNA) Program eligibility

Qualifications- Active Washington State Certified Nursing Assistant (CNA) license preferred but not required
- BLS certification every 2 years; will help obtain
- Completion of a 10-week Community Health Worker Program; will help obtain
- Requires travel to and from Clinics and patient homes
Responsibilities- Build supportive relationships with patients and families
- Assess social needs and connect patients to community resources
- Support access to food, housing, transportation, and healthcare services
- Provide follow-up through calls, visits, and care coordination
- Collaborate with care teams to address patient goals and barriers
- Advocate for patients and serve as a liaison with community agencies
- Document patient interactions and progress in the EMR
- Manage caseload, attend meetings, and maintain HIPAA compliance
