Job Title: Medical Social Worker (MSW) - Part-Time
Location: St. Louis, MO (Field-based) Pay: $70.00 per visit Schedule: Part-time (Approximately 6 visits per week)
About the Role
Are you a compassionate MSW looking to make a tangible difference in the lives of the elderly? We are seeking a dedicated Medical Social Worker (MSW) to join our team. At Breakthrough Healthcare, we believe our clients are family. Inspired by a legacy of caregiving, we provide more than just medical services-we provide dignity, advocacy, and emotional support to clients in the comfort of their own homes.
Key Responsibilities
  • Initial Assessments: Lead the evaluation of clients' social and emotional needs and develop comprehensive Plans of Care (POC).
  • Advocacy & Counseling: Counsel clients and families on coping with the social and emotional impacts of illness.
  • Supervision: Provide clinical oversight and co-signing for Bachelor's level social workers (LSWs/BSWs).
  • Community Connection: Identify and link clients with appropriate community resources to ensure their safety and well-being.
  • Care Coordination: Collaborate with physicians and the clinical manager to report changes in condition and ensure seamless care.

Qualifications
  • Education: Master's Degree from a school accredited by the Council on Social Work Education (CSWE).
  • Licensure: Current state licensure in Missouri (and/or Illinois, as we expand).
  • Experience: Minimum of one year of social work experience in a healthcare setting (home health experience preferred).
  • Skills: Strong analytical and interpersonal skills; ability to work independently in a field-based environment.
