The Addiction Health Center, located in Islip, New York, is actively seeking a dedicated and compassionate Licensed Clinical Social Worker (LCSW) to join our committed team. Work Environment

This position offers flexibility through a fully remote work setting, allowing you to conduct sessions and manage cases efficiently from the comfort of your own home. Our operational hours are from Monday to Friday, 9:00 AM to 5:00 PM, providing a structured schedule that promotes work-life balance and encourages professional dedication.

Key Responsibilities

• Act as a liaison between patients, families, and outpatient providers to ensure seamless care coordination, fostering open communication among all stakeholders.
• Utilize clinical expertise to interact with insurance providers for pre-certification and utilization management, ensuring patients have access to necessary services.
• Engage actively in discharge and aftercare planning to support clients transitioning out of treatment facilities or significant interventions.
• Provide therapeutic meal support, modeling a healthy relationship with food while assisting clients in developing positive coping strategies.
• Deliver safe, empathetic, and affirming care tailored to the unique circumstances and identities of each client, especially considering the intersectionality of their backgrounds and experiences.

Patient Population / Client Focus

Our client focus is primarily on adults who are confronting the challenges of addiction, as well as co-occurring disorders. You will work with individuals from various backgrounds, providing culturally competent care designed to meet their unique needs and support their journey toward recovery. Understanding and addressing the complex interplay of mental health and addiction will be crucial in your role as LCSW.

Qualifications

• A Master's degree in Social Work from an accredited institution is required.
• Possession of a valid LCSW license in the state of New York.
• Prior experience working with older adults and individuals experiencing substance use disorders is essential.
• Proven clinical and diagnostic skills, accompanied by the ability to rapidly assess and respond to client needs.
• Demonstrated strength in communication and interpersonal skills, enabling effective collaboration with colleagues and clients alike.
• Capacity to work both independently and as part of a dynamic team focused on achieving high-quality client outcomes.

Preferred Experience

• Experience in a community health setting or clinical environment that emphasizes addiction recovery and dual diagnosis treatment.

Compensation and Benefits

We offer a competitive salary range between $90,000 and $105,000 per year, commensurate with experience and qualifications.
