This position offers a flexible, remote work setting, allowing you to engage with clients from the comfort of your home or dedicated telehealth space. Key Responsibilities
  • Act as a liaison between clients, families, and outpatient providers, ensuring clear communication and coordinated care.
  • Engage with insurance companies to facilitate pre-certification and management of utilization for services provided.
  • Assess individual needs and craft personalized discharge and aftercare plans in collaboration with the clinical team.
  • Provide therapeutic meal support, modeling a healthy relationship with food and encouraging positive eating habits.
  • Deliver safe, empathetic, and affirming care tailored to the diverse identities and experiences of each client.
  • Document and maintain comprehensive records of client interactions, treatment plans, and progress according to regulatory guidelines.

Qualifications
