YSS is looking for a passionate, people-centered leader to serve as our Youth Transition Decision-Making (YTDM) Statewide Coordinator — someone who believes strong support systems can change the trajectory of a young person’s life.
In this role, you’ll lead statewide coordination efforts that help youth and young adults successfully transition into adulthood through the Iowa Aftercare Services Network (IASN). From overseeing YTDM facilitators and referrals across Iowa to strengthening partnerships with agencies and community providers, you’ll play a key role in helping youth feel supported, connected, and empowered during some of life’s most critical transitions.

You’ll help ensure youth and families receive timely, trauma-informed support while guiding facilitators, coordinating services, and building strong partnerships that improve outcomes for young people across the state.
What You'll Do...
Program Leadership & Oversight
- Oversee and support YTDM Facilitators statewide.
- Lead facilitator team meetings, supervision, and ongoing training efforts.
- Ensure compliance with YTDM policies, procedures, timelines, and documentation standards.
- Oversee facilitator billing, database training, and data entry processes.
Referral & Case Coordination
- Serve as the primary contact and gatekeeper for YTDM and Pre Aftercare referrals within assigned coverage areas.
- Maintain accurate tracking systems for referrals, scheduled meetings, documentation, and service data.
- Support facilitators in connecting youth and families to appropriate community resources and services.
Collaboration & System Coordination
- Build and maintain partnerships with HHS, IASN, TPS, WWA, Juvenile Court Services, and other community partners.
- Collaborate with YSS, IASN, and HHS leaders to develop and maintain effective systems and processes.
- Attend required provider panel meetings, network meetings, supervision sessions, and YSS Aftercare team meetings.

Required Qualifications - MUST HAVES:
- Bachelor’s degree in Human Services or a related field.
- At least three years of experience working with high-risk youth or young adults.
- At least one year of case management experience.
- Prior supervision, leadership, or team coordination experience.
- Travel, including transportation of clients, is required for this position. For YSS insurance purposes, you must:
- be 21 years of age,
- have a current, valid, state-issued, driver’s license, and
- have a clean driving record.
Preferred Qualifications - SHOULD HAVES:
- Experience coordinating services across teams or community partners.
- Experience in aftercare, child welfare, juvenile justice, or human services.
- Experience with crisis intervention and de-escalation.
- Knowledge of trauma-informed and youth-centered practices.
- Proficiency with Microsoft Office and web-based systems.
- Strong communication and relationship-building skills.
- Strong organization, multitasking, and time management skills.
- Strong critical thinking and problem-solving abilities.
