Job Summary
Provide outpatient physical therapy services through patient assessment, treatment planning, intervention, and ongoing evaluation. Collaborate with physicians and rehabilitation staff while supervising Physical Therapist Assistants and rehabilitation aides to deliver high-quality patient care.
Responsibilities
Perform comprehensive patient evaluations and functional assessments.
Develop individualized treatment plans based on patient goals and clinical findings.
Update plans of care as patient status changes.
Coordinate treatment frequency and duration with patients and the rehabilitation team.
Educate patients and caregivers on diagnoses, prognosis, home exercise programs, and treatment plans.
Supervise Physical Therapist Assistants and rehabilitation aides.
Maintain accurate and timely documentation.
Required Qualifications
Active Michigan Physical Therapist License
Current AHA BLS Certification
Previous Physical Therapist experience
Outpatient Physical Therapy experience
Current PT skills checklist (within 1 year)
Two supervisory references completed within the last 2 years
Must reside more than 50 miles from the facility (no local candidates)
Preferred Qualifications
Acute Care experience
Strong orthopedic and functional rehabilitation background
Experience in a multidisciplinary outpatient setting
