City/StateVirginia Beach, VAWork ShiftFirst (Days)Overview:Sentara Service Line Administration is now hiring a full-time Cardiac Care Continuum Navigator in Virginia Beach, VA!HoursMonday-Friday 8:00AM - 5:00PMThis is a hybrid position, offering the flexibility to work both remotely and in person. This position requires three days per week on site and two days remotely (at the candidate's discretion).OverviewThe SR. Navigator is responsible for maintaining and reviewing quality metrics and fallouts, program development, participating in system initiatives, data analysis, maintaining programs and performing duties appropriate to the navigator’s assigned service line. The SR. Navigator on-boards new team members and provides guidance and mentorship to other navigators.Daily activities include those duties required to ensure the ongoing operational and managerial elements of the program are running smoothly. Daily activities typically account for 60% of the SR. Navigators time and include: daily rounds, discharge planning, physician relations, pre-operative chart/pre-admission testing process and monitoring the destination center surgery schedule. Program development activities include the activities necessary to ensure the long-term success and growth of the program.
