Job description

Job Title: Social Worker - Hospital Pathway
Location: Sutton
Pay Rate:  32 per hour

Job Description:

Liquid Personnel is currently recruiting a skilled and compassionate Social Worker to join the Hospital Pathway Adult Social Work Team. The successful candidate will play a critical role in supporting adults during their transition from hospital care to community-based services, ensuring a smooth and effective discharge process.

What will your responsibilities be?

  • Conduct comprehensive assessments of patients' needs and develop individualized discharge plans in collaboration with healthcare professionals, patients, and their families.
  • Manage a caseload of patients, ensuring timely and coordinated discharge from the hospital to appropriate community or residential settings. Identifying and addressing any safeguarding concerns to protect vulnerable adults from abuse, neglect, or exploitation
  • Provide ongoing support and advocacy for patients, ensuring their rights and needs are prioritized throughout the discharge process.

Qualifications & Experience:

To be successful in this role you must have, 

  • Social Work England registration
  • Eligible to work in the UK
  • Hold a full UK driving licence
  • Post-qualified experience
  • Degree level or equivalent in Social Work
