Job Description:
Job Summary
Responsible for delivering compassionate, patient-centered care in the home setting through assessments, treatments, and coordination of individualized nursing services. Collaborates with interdisciplinary team members to implement and evaluate care plans that promote recovery, independence, and optimal health outcomes. Ensures adherence to clinical standards, safety protocols, and regulatory requirements while maintaining accurate documentation. Educates patients and caregivers to support effective disease management and continuity of care. Fosters trust, communication, and quality service to enhance the home health experience.
Essential Functions
• Conducts comprehensive patient assessments, monitors health status, and documents findings accurately.
• Administers medications, wound care, and prescribed treatments under appropriate supervision and within the LPN scope of practice.
• Implements nursing care plans in collaboration with registered nurses, therapists, and other healthcare professionals.
• Educates patients and caregivers on medication management, disease prevention, and self-care techniques.
• Coordinates care visits, communicates patient progress, and reports changes in condition to supervising clinicians.
• Ensures infection control, safety compliance, and proper handling of medical equipment in the home environment.
• Maintains accurate, timely medical records in accordance with agency and regulatory standards.
• Supports quality improvement initiatives, patient satisfaction efforts, and interdisciplinary collaboration.
• Participates in ongoing professional development and maintains required licensure and competencies.
• Performs other duties as assigned.
Education Requirement
High school diploma or equivalent and vocational/technical training in relevant area.
