Job Summary
We are seeking a compassionate and experienced Registered Nurse (RN) to join our Home Healthcare team. In this role, you will be responsible for creating, implementing, and managing individualized plans of care for patients in their homes. The ideal candidate has prior home healthcare experience, strong clinical judgment, excellent communication skills, and a genuine passion for patient-centered care.
This position requires travel to patient homes across Atlanta, Cobb, Gwinnett, Henry, and Clayton Counties. The RN will play a vital role in coordinating care, monitoring patient conditions, and ensuring high-quality clinical outcomes in a home-based setting.
Responsibilities
  • Conduct comprehensive patient assessments in the home setting
  • Develop, implement, and update individualized plans of care
  • Coordinate care with physicians, caregivers, and interdisciplinary team members
  • Administer medications and treatments as prescribed
  • Monitor patients for changes in condition, side effects, or adverse reactions
