Position Summary:

We are seeking a motivated Occupational Therapist (OT) to join our home health team. In this role, you will assess functional needs and deliver in-home occupational therapy services to adults and older adults with a variety of physical, cognitive, and environmental challenges. Your interventions will help patients achieve greater independence in their daily lives.


Key Responsibilities:
  • Conduct initial assessments and create customized plans of care for each patient

  • Deliver evidence-based occupational therapy interventions for ADL/IADL limitations, home safety, fall risk, and cognitive function

  • Collaborate with physicians, nurses, physical therapists, speech therapists, and care coordinators

  • Provide family and caregiver training on therapy goals and safety strategies

  • Document evaluations, progress notes, and reassessments in the EMR system (Your Tailor Made Senior Services)

  • Update care plans regularly based on patient progress and changing needs

  • Comply with agency policies, Medicare regulations, and professional standards of care

  • Supervise Occupational Therapy Assistants (COTAs) as needed

  • Participate in interdisciplinary meetings and case conferences

  • Promote occupational therapy education in the community, if applicable


Requirements:
  • Current Occupational Therapy license in the State of Texas (Required)

  • Graduate of an ACOTE-accredited Occupational Therapy program

  • At least 1 year of OT experience (Home health or geriatrics preferred)

  • Strong clinical judgment and independent decision-making skills

  • Reliable transportation, valid driver’s license, and proof of insurance

  • Comfortable using EMR and mobile documentation tools
