What You'll Do
  • Conduct telehealth physical therapy sessions from home, providing virtual evaluations, follow-up care, patient education, and treatment guidance
  • Deliver hands-on, in-person physical therapy care during scheduled onsite visits
  • Evaluate patient needs and develop individualized treatment plans focused on improving mobility, strength, safety, function, and overall quality of life
  • Collaborate closely with interdisciplinary care teams, physicians, caregivers, and facility staff to ensure coordinated care
  • Complete accurate and timely documentation in compliance with company, payer, and regulatory standards
  • Monitor patient progress, adjust treatment plans as needed, and advocate for positive patient outcomes
  • Utilize technology confidently to create seamless telehealth experiences for patients
  • Help support a positive, patient-centered therapy culture rooted in service excellence

What We're Looking For
  • Licensed Physical Therapist (PT) or license eligible in the state of practice
  • Graduate of an accredited Physical Therapy Program
  • Interest or experience in geriatric rehabilitation or senior living therapy
  • Passion for patient-centered care and improving quality of life for older adults
  • Excellent communication and interpersonal skills
  • Strong time management and organizational abilities
  • Comfortable using EMR/documentation systems and technology platforms
  • Ability to work independently while remaining highly collaborative
