DaVita is seeking a full-timeNurse Practitioner (NP) Managerto join ourIKCCare.This role provides clinical leadership and management for Nurse Practitioners within a designated region, ensuring the delivery of high-quality, cost-effective care for patients withChronic Kidney Disease(CKD) and End-Stage Kidney Disease (ESKD).

The NP Manager functions autonomously while partnering closely with clinical and operational leadership to support population health management, quality outcomes, and the overall financial sustainability of the program.

Work Schedule & Environment

• Monday-Friday schedule

• Hybrid role with work-from-home and regional travel responsibilities as this program will cover several states

• Willingness to travel regionally to support meetings,trainings, and clinical initiatives

Key Responsibilities

Clinical Leadership & Team Management

• Serve as the clinical leader for a team of Nurse Practitioners, providing oversight, coaching, supervision, and performance management.

• Ensurehigh standardsof clinical quality and productivity, includingaccuratedocumentation,appropriate diagnoses, closure of Plan of Care gaps, and compliant coding practices.

• Support NP hiring efforts and oversee onboarding of new NPs. Provide ongoing clinical guidance and training to NPs to improve understanding of CKD, ESKD, and commonly associated chronic conditions.

• Provide ongoing clinical education and guidance to enhance NPexpertisein CKD, ESKD, and commonly associated chronic conditions.

Strategy, Quality & Population Health

• Partner with Medical Officers and Operations Leadership to develop and execute clinical and operational strategies.

• Identifyopportunities to streamline workflows and prioritize care for high-risk, high-utilization patient populations.

• Lead complex case reviews and manage escalations involving high-risk or high-utilization patients, collaborating with interdisciplinary teams and external resources as needed.

• Participate in population health initiatives and contribute to quality improvement efforts,outcomestracking, and performance measurement.

Compliance & Program Support

• Ensure compliance with DaVita policies and procedures, as well as all applicable state and federal regulations, including CMS, HIPAA, OSHA, and accreditation standards (e.g., NCQA).

• Support market Comprehensive Health Evaluations as needed.

• Performadditionalduties as assigned.

Preferred Qualifications & Experience

• Master's degree in Nursingwith current Nurse Practitioner licensure.

• 4-6years of Nurse Practitioner experience; experience in population health, primary/family care, and/ornephrology stronglypreferred.

• At least 2 years of leadership or supervisory experience.

• Experience with Medicare Advantage risk adjustment, documentation, coding, and Health Risk Assessments for high-risk or chronically ill populations.

• Demonstrated clinicalexpertisein assessment, diagnosis, care planning, implementation, documentation, and evaluation for chronic disease management.

• Strong verbal and written communication skills.

• Ability to handle confidential information with discretion and sound judgment.

• Proficiencyin Microsoft Word, Excel, and Outlook; intermediate computer skillsrequired.

• Excellent organizational skills with strong attention to detail.

The Salary Range for the role is $141,000.00-$175,000.00 per year.If a candidate is hired, they will be paid at least the minimum wage according to their geographical jurisdiction and the exemption status for the position.New York Exempt: New York City and Long Island: $66,300.00/year, Nassau, Suffolk, and Westchester counties: $66,300.00/year, Remainder of New York state: $62,353.20/year New York Non-exempt: New York City and Long Island: $17.00/hour, Nassau, Suffolk, and Westchester counties: $17.00/hour, Remainder of New York state: $16.00/hourWashington Exempt: $80,168.40/year Washington Non-exempt: Bellingham: $19.13/hour, Burien: $21.71/hour, Everette: $20.77/hour, Unincorporated King County: $20.82/hour, Renton: $21.57/hour, Seattle: $21.30/hour, Tukwila: $21.65/hour, Remainder of Washington state: $17.13/hour
