Your Responsibilities
  • Deliver Patient-Centered Care
    • Assess and evaluate patients’ functional abilities, mobility, strength, and rehabilitation potential.
    • Create and execute personalized care plans in collaboration with physicians and the care team.
    • Administer hands-on therapy to enhance mobility, strength, balance, and independence.
    • Educate patients and their caregivers on treatment protocols, exercises, and safety measures.
  • Drive Outcomes & Recovery
    • Track patient progress and modify treatment plans as necessary.
    • Facilitate safe transitions of care and assist in discharge planning.
    • Identify additional patient needs and work with interdisciplinary teams.
  • Collaborate & Communicate
    • Work closely with nurses, therapists, and physicians to ensure cohesive care.
    • Accurately document patient care and progress in the electronic medical record (EMR).
    • Engage in case conferences and team meetings.
  • Lead & Support
    • Oversee Physical Therapist Assistants (PTAs) in line with state regulations.
    • Provide direction and education to caregivers and support staff.
Your Qualifications
  • Graduation from an accredited Physical Therapy program (DPT or Master’s).
  • Current PT license in the state where you practice.
  • 1–2 years of experience, preferably in home health.
  • Strong clinical assessment and critical thinking capabilities.
  • Excellent communication skills and ability to educate patients.
  • Current CPR certification.
  • Valid driver’s license, dependable transportation, and auto insurance.
