Description
Providence House Assisted Living in Brighton is looking for an upbeat, warm and caring wellness nurse for to join our wellness team!
We are a 102 unit building made up of 90 traditional apartments and a 12 unit memory care program.
You provide outstanding patient care and physical support for residents on a daily basis. You work well as a member of a team, are organized & dependable with an enthusiastic and cheerful disposition.
The Wellness Nurse performs multiple care duties that contribute to the health, well-being, and quality of life for the residents of Providence House. The schedule is flexible with some weekends and occasional holidays.
Essential Responsibilities:
-Works in the provision of care and services to residents
-Assisting the management of the wellness program
-Assists in the assessment of potential residents according to the assessment policy in a timely manner and assist with the establishment of a resident service plan
-Assists with updating resident service plans at least every 6 months or when circumstances change
-Maintain confidential medical files of all residents
-Provide health & resource information for residents & their families
-Coordination and oversight of SAMM as directed by EOEA guidelines
-Entry and update information in Eldermark Database for residents
Qualifications:
-Graduate of Commonwealth of MA approved School of Nursing, with current MA registration as a LPN or RN required
-Geriatric experience in a clinical and health care setting preferred
-Ability to speak, read and write effectively in English
We offer a salary of $40.00 - $45.00 per hour based on experience. We offer benefits including health, 401k with employer match, paid time off and employee referral program!
