PART TIME AND ON-CALL POSITIONS AVAILABLE
Nikkei Manor Assisted Living, located in Seattle's International District, is seeking a compassionate and dedicated Wellness Nurse to join our team and provide exceptional care to our residents.
As a Wellness Nurse, you will play a key role in supporting the health and well-being of our residents. You'll use your clinical expertise to perform ongoing assessments, monitor health conditions, and ensure each resident receives the highest quality of care in a supportive, home-like environment.
Our nurses are part of a collaborative care team that includes Caregivers, Medication Technicians, and our Health Services Director, all working together to promote an atmosphere of dignity, safety, and wellness for our residents.
Requirements
Essential Duties and Responsibilities
โ€ข Evaluate resident health status and follow through with appropriate treatment and or documentation.
โ€ข Provide residents' medications and treatments that require administration by a licensed nurse.
โ€ข Monitor and maintain records reflecting residents' physical and mental condition.
โ€ข Coordinate and oversee medication and treatment services, ensuring accurate and complete daily documentation.
โ€ข Provide support and consultation to the Resident Assistants and Med Techs in problem-solving, service challenges, and resident health issues.
โ€ข Act as a liaison between residents, doctors, and families regarding medical and social concerns.
โ€ข Coordinate with residents and families on outside appointments and arrange KNW transportation if necessary.
โ€ข Maintain current medication orders for residents.
โ€ข Coordinate with Health Services Director for Assessments of potential residents prior to move-in, residents returning from hospital or skilled nursing facility for appropriate care and perform ongoing assessments to determine the need for additional or modified services.
โ€ข Establish and maintain current knowledge of the requirements of regulatory agencies.
โ€ข Respond to emergency situations and administer first aid as needed.
โ€ข Investigate and document resident incidents
โ€ข Assist HSD to monitors resident incident reports and medication error reports to assess causes and preventative measures.
โ€ข Assist Health Services Director with employee evaluations.
โ€ข Attend mandatory in-services and training.
โ€ข Handle all forms of resident Protected Health Information (PHI) in a confidential, private and secure manner.
โ€ข Assist in instituting infectious illness outbreak-management with guidance from Wellness Director and Public Health
โ€ข Demonstrates familiarity with EHR systems (Currently using PCC)
โ€ข Arrange and coordinate Home Health, Hospice, and Respiratory Services
โ€ข Performs other duties as assigned.
Supervisory Responsibilities
This position does not have direct supervisory responsibility, but provides shift supervision of Resident Caregivers and Medication Technicians and serves as a delegate supervisor in the absence of the Health Services Director.
Qualifications
To perform this job successfully, an individual must be able to perform each essential duty satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
Must satisfy all state health, licensure/certification, and background requirements.
Education/Experience: must be a licensed or registered nurse. 2-4 years of nursing experience, including at least one year of geriatric nursing is preferred
Knowledge, Skills and Abilities:
โ€ข Relationship to the local Japanese American community preferred
โ€ข Ability to read/speak/write Japanese preferred
โ€ข Must be self-motivated.
โ€ข Must be able to relate to residents and staff of diverse backgrounds.
โ€ข Must be able to use tactful, diplomatic communication techniques in potentially sensitive or emotional situations.
โ€ข Must be able to work flexible hours.
โ€ข Demonstrates an appreciation of the heritage, values and wisdom of the residents and an understanding of the aging process.
Certificates and Licenses: Currently licensed or eligible for license as LPN/LVN or RN. CPR and First Aid certifications must be completed if not current, plus any additional licensing requirements in state(s) of practice, Food Handler Card.
Communication Skills: Receive and carry out verbal or written instruction. Communicate effectively in person, over the phone, or via email with customers and co-workers. Write and proofread routine correspondence and reports with appropriate grammar and spelling. Make group presentations to share information or explain policies and procedures.
Organization/Self-Management Skills: Perform a variety of duties, moving between tasks of differing nature, with occasional distractions and interruptions. Prioritize competing requirements to complete work in a timely manner when there are changes to workload and assignments. Work independently and follow through on assignments with minimal direction.
Computer Skills: Proficiency with computers, including Windows operating system, Microsoft Office Suite, and EHR system.

Physical Demands
Constantly stands and walks around the community, interacting with residents, coworkers, and others. Uses hands and fingers to feel, handle, and operate supplies, controls, tools, and objects, and hands and arms to reach. Frequently communicates with others to exchange information in person, and over the phone, and must be capable of effective communication in these situations. Occasionally moves items weighing up to 25 pounds and may assist in transferring residents weighing more than 100 pounds. Frequently required to bend, twist, stoop, kneel, crouch, or crawl.
Work Environment
This position is most frequently situated inside of a climate-controlled assisted living community, with a low to moderate noise level. In certain circumstances, a staff member may be exposed to communicable diseases and infections, other biological hazards, and various drugs and chemicals. Due to these potential hazardous exposures, you may be required to wear Personal Protective Equipment (PPE) in addition to universal precautions. PPE could include, but is not limited to, gloves, gowns, masks, respirators, and goggles.
