Job Details: Location—YMCA, Greencastle, IN 46135. Position Type—Part Time. Job Shift—Day.

About Us: Putnam County Occupational Medicine and My Wellness located in the YMCA in Greencastle are dedicated clinics committed to providing primary care and occupational medicine support to our community. We pride ourselves on personalized care, patient education, resource accessibility, and delivering as much care as possible in-house. Job Description: We are seeking an enthusiastic and compassionate Family Medicine Practitioner—either a Nurse Practitioner (NP) or Physician Assistant (PA)—to join our team. The ideal candidate is passionate about rural healthcare, efficient, resourceful, and dedicated to delivering the highest level of patient care.

Key Responsibilities
  • Provide comprehensive primary care services to patients in a rural setting.
  • Educate patients and families about health conditions, treatment plans, and preventive care.
  • Assist patients in navigating healthcare resources, ensuring they receive needed care either in‑house or through referrals.
  • Provide in‑house lab testing and perform a variety of procedures to meet patient needs.
  • Collaborate with the healthcare team to deliver holistic, patient‑centered care.
  • Maintain accurate and detailed patient records, ensuring compliance with all regulations and standards.
  • Contribute positively to our family‑oriented workplace culture through collaborative teamwork.

Qualifications
