Wound Care Advanced Practice Clinician

Position Highlights:

As part of the Wound Care Team the Wound Care Advanced Practice Clinician will provide excellent medical, diagnostic, therapeutic care, education, and preventive services to all patients. The Wound Care provider will evaluate and treat chronic and complex wounds and edema caused by disease, accident, or surgical incision. The Wound Care provider will monitor patient progress throughout the healing process and educate the patient on the care of wounds outside of the clinical setting to achieve the best possible healing outcomes. The Wound Care provider will provide services in accordance with evidence-based guidelines, applicable federal, state, regulatory and company standards. The Wound Care provider's performance will be tracked and comparatively reported using metrics related to the strategic direction and operational needs of the organization.

The Wound Care provider will:

  • Provide diagnosis, treatment, and management of patients according to current standards of care in both outpatient and home settings.
  • Advocate for patients and their families in providing them resources for care.
  • Track patient status and adjust wound management according to evidence-based guidelines.
  • Utilize managed care processes for cost-effective and efficient use of lab, radiology, and extended service providers, including: specialists, hospitals, skilled nursing providers, and other ancillary services
  • Complete timely and comprehensive medical documentation in EMR systems according to company standards
  • Participate with peers, employed and DNG PCPs, for the comprehensive care of the WellMed patient.
  • Conduct or directs education to patients, families, and other health care providers
  • Conduct educational activities for organization's physicians, physician assistants, nurse practitioners, and their ancillary staff for the purposes of education and patient care.
  • Participate in business and patient care meetings
  • Perform all other related duties as assigned

Required Qualifications:

  • Certification through the American Academy of Nurse Practitioners (AANP), American Nurses Credentialing Center (ANCC), or the National Commission on Certification of Physician Assistants (NCCPA)
  • Current Licensure through the Texas Board of Nurse Examiners OR Physician Assistant license by the Texas State Medical Board
  • Valid DEA license or the ability to obtain one prior to employment

Preferred Qualifications:

  • A working understanding and support of a managed care environment
  • Bilingual (English/Spanish) fluency

Compensation for this specialty generally ranges from $109,500 - $164,000. Total cash compensation includes base pay and bonus and is based on several factors including but not limited to local labor markets, education, work experience and may increase over time based on productivity and performance in the role. We comply with all minimum wage laws as applicable. In addition to your salary, we offer benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
