REQUIRED QUALIFICATIONS:

  • Active Oregon license as a Nurse Practitioner (FNP, AGNP, or AGNP-PC), PA-C, MD, or DO (or eligibility to obtain)
  • Board certification in Family Medicine, Internal Medicine, or relevant specialty
  • Strong interest in preventive, functional, and lifestyle medicine
  • Excellent communication and patient relationship skills
  • Comfortable working in a fast-paced, collaborative environment
  • Proficient with electronic health records (NextGen experience a plus)
