Job description

The RN would be on call from Friday at 4:30 pm until Monday at 8:00 am (eastern). The RN would be responsible to cover any as-needed visits that arise, as well as scheduled weekend visits including admission visits. Compensation will include an on-call stipend PLUS pay per visit for each visit completed. The Nurse will be responsible for assessing patient health problems and needs, developing and implementing nursing care plans, and maintaining medical records. May advise patients on health maintenance and disease prevention or provide case management. Licensing or registration is required.
ESSENTIAL DUTIES AND RESPONSIBILITIES
The essential functions include, but are not limited to the following:

  • On call from Friday at 4:30 pm until Monday at 8:00 am (Eastern)
  • Must cover any as-needed visits that arise, as well as schedule weekend visits, including admission visits
  • Provide skilled nursing treatments as ordered by the client's doctor
  • Complete evaluation tasks, including vital signs and medication reviews
  • Coordinate with physicians, therapists, and other individuals in the client's care plan
  • Assess and chart observations of the client's condition at each visit
  • Maintain complete and accurate documentation of nursing care
  • Work collaboratively with all members of the healthcare team
  • Provide education to clients and families on proper home health care procedures and strategies
  • Record patients' medical information and vital signs
  • Monitor, record, and report symptoms or changes in patient's conditions
  • Administer local, inhalation, intravenous, or another anesthetic
  • Prepare patients for and assist with examinations or treatments
  • Modify patient treatment plans as indicated by patients' responses and conditions
  • Order, interpret, and evaluate diagnostic tests to identify and assess patient's condition
  • Monitor all aspects of patient care, including diet and physical activity
  • Engage in research activities and participation in formal educational training activities related to nursing
  • Perform other duties as assigned
MINIMUM QUALIFICATIONS (KNOWLEDGE, SKILLS, AND ABILITIES)
  • Must be available on weekends from Friday 4:30 pm until Monday 8:00 am (Eastern)
  • Associates degree required; Graduate of an accredited state-approved, Bachelor's Degree from a school of nursing preferred
  • Current and valid California Registered Nurse License
  • Prior nursing experience, specifically in home health (preferred)
  • Current CPR certification (required)
  • Driver's License

  • Strong interpersonal skills that build personal rapport and establish trust with the client
  • Computer literate
  • Strong organizational skills
  • Excellent written and communication skills
  • Ability to meet physical demands of the job (ex: lifting 50 pounds for transfers)
