Job description

Weekend Supervisor position

Full-time (80 hours biweekly)

12 hours Friday, Saturday, and Sunday and 4 hours Monday 

6 weekends off a year

On call coverage over weekend

**Can be located in WellSpan's East, Central, or West Regions - this includes Adams, Franklin, Lancaster, Lebanon, and York counties!!

General Summary

Responsible for delivery and evaluation of patient care services throughout the WellSpan VNA service area. Assists in implementing new programs as well as evaluating and updating current programs and policies. Assumes the responsibilities of the Clinical Manager as designated.

WellSpan VNA Home Health Services

For more than 100 years, WellSpan VNA Home Care has understood that the best place to treat the body is where the heart is - at home.  Our specialized staff of nurses, aides, therapists and caregivers work with physicians to provide the services necessary to treat patients in the comfort of their own home. 

WellSpan VNA Home Care provides easy access to excellent care.  We serve Central and South Central PA including:  Adams, Berks, Chester, Lancaster, Lebanon, York and Franklin counties.

Qualifications
Minimum Education:

  • Associates Degree Nursing Required or
  • Diploma Program Nursing Required
  • Bachelors Degree Nursing (BSN) or rehabilitation therapy. Preferred

Work Experience:

  • 3 years Clinical nursing or therapy experience. Required
  • 3 years Clinical nursing or therapy experience with an emphasis in home care. Preferred

Licenses:

  • Licensed Registered Nurse Upon Hire Required or
  • Registered Nurse Multi State License Upon Hire Required or
  • Licensed Physical Therapist Upon Hire Required or
  • Licensed Occupational Therapist Upon Hire Required or
  • Licensed Speech-Language Pathologist Upon Hire Required and
  • Heartsaver CPR/AED Upon Hire Required

Driver's License Statement:

Must possess a valid driver's license in the current state of residence. Driving record must meet requirements established by WellSpan Risk Management.

Duties and Responsibilities
Essential Functions:

  • Related to Patient Care
    * Follows Conditions of Participation to ensure all regulations are followed under Medicare.
    * Ensures that quality and efficient care is provided to all patients through 5 elements of documentation, best practices, and escalated change of status by clinicians.
    * Utilizes SHP clinician scorecard to ensure quality score ratings and rehospitalization rates.
    * Monitors productivity of respective staff and provides additional guidance when staff fall below expectations.
    * Reviews appropriate documentation through auditing for content, accuracy, and completeness and ensures completion within the time frame required.
    * Identifies un-met patient needs and reports findings to appropriate Clinical Manager.
    *Completes S1 and CRS as needed. Completes 4C with team members to identify root cause and countermeasures.
    * Holds In Episode Management meetings on a regularly scheduled basis with individual staff to ensure safe quality care, progress towards goals, and proper utilization of services.
    * Provides after-hours and on-call staff support as well as visits and consultation on a scheduled, rotating basis. Makes home visits as required/needed (Expectation - 25-50% time spent with staff in field or training environment).
    * Ensures all information is communicated accurately and consistently.
    * Provides clinical assistance to respective scheduler as requested. Informs scheduling department of any clinical patient/staff related issues in the event patient care is/could be impacted.
    * Demonstrates leadership by example while promoting "Just Culture" and positive interactions throughout the organization.
  • Related to Staff
    * Completes/assists in performance appraisals for staff using available performance data within the VNA Bold Goals.
    * Establishes/assists in development plans for all respective staff. Ensures clinical competencies of all field staff members.
    *Completes home visits to mentor, train, and assess individual clinician's job performance and skill competency and to evaluate patient satisfaction, workflows and compliance.
    * Promotes open and effective communication throughout the team, organization, and System.
    * Provides positive leadership to all staff and demonstrates support of the organization.
  • Related to Home Health Management/Hospice
    * Participates in the planning, implementation, management and evaluation of the Home Health & Hospice Services Program as requested.
    * Participates in the identification and development of new clinical policies and the regular review and updating of existing clinical policies as indicated.
    * Coordinates services with other community agencies as directed.
    * Participates on special committees as requested.
  • Related to Corporate Management
    * Provides assistance in meeting all educational needs of clinical staff on the team as requested.
    * Represents WellSpan VNA Home Care in a professional manner.
    * Responsible for efficient utilization of all resources including, but not limited to, materials/supplies, equipment, time, dollars and FTEs.
    * Seeks consultation/advisement as needed.
