Primary Care Functional Medicine - Nurse Practitioner

We are seeking a passionate and skilled Medical Provider to join our growing team. You will provide comprehensive medical services, foster lasting relationships with your patients, and collaborate with a high-functioning care team to drive optimal health outcomes.

We are actively hiring for our Southern Oregon clinics in Talent, Medford, Central Point, and Grants Pass

Compensation: Guaranteed salary with earning potential of $300k+, relocation offered

Responsibilities and Duties:

  • Provide direct patient care through office visits, telehealth, and occasional same-day appointments
  • Perform comprehensive assessments, order and interpret labs/imaging, and diagnose acute and chronic conditions
  • Develop personalized treatment plans that may include lifestyle interventions, supplements, hormone therapies, and prescriptions
  • Educate and empower patients on preventive health and functional medicine principles
  • Collaborate with internal health coaches, nutritionists, mental health providers, and clinical support staff
  • Document visits in the EHR in a timely and compliant manner
  • Participate in team meetings, quality improvement initiatives, and training sessions
  • Support clinic operations by contributing to a positive, team-oriented environment

Required Qualifications:

  • Active Oregon license as a Nurse Practitioner, PA-C, MD, or DO (or eligibility to obtain)
  • Board certification in Family Medicine, Internal Medicine, or applicable specialty
  • Strong interest in preventive, functional, and lifestyle medicine
  • Excellent communication and patient relationship skills
  • Comfortable working in a fast-paced, collaborative environment
  • Proficient in using electronic health records

Nurse Practitioner Licenses Accepted:

  • FNP- Family Nurse Practitioner
  • AGNP- Adult Gerontological Nurse Practitioner
  • AGNP- PC- Adult Gerontological Nurse Practitioner
