CT/CV PA locum in Toledo, OH-Asap and Ongoing-OH license required to apply-$140 Hr-Scheduled and Call available
Job Details:

Coverage Location: Mercy St. Vincent, Mercy St. Anne, Mercy St. Charles, Mercy Perrysburg (Provider will be receiving privileges for ALL locations listed)

  • Dates of Coverage: 10 days/month covering over two weekend. January-April then TBD
  • Reason for Coverage: Open position. Training new hire
  • Practice Setting: Office hours inpatient post op floor and ICU care
  • Schedule (Shifts/Hours): 8:00 -4:00 M-F
  • Call Schedule: 10 days of call/month
  • Call Ratio: 1:3
  • Average Number of Patients Per Day: 12
  • Documentation System/EMR: EPIC 
  • Trauma Level/Designation: Level 1
  • Will APP be Working Autonomously: Yes
  • Supervising Physician Present During Assignment: Physician will round daily and be available 24/7 by phone
  • Required Duties: Suture Removal, Diagnose Illnesses, Treat Illnesses, Order Tests, Interpret Tests, Write Prescriptions, Coordination of Care, Rounding, Pre-Op Physical Exams
  • Patient Age Demographic: (Adolescent, Child, Adult, Geriatric): Adult/geriatric
  • Estimated Credentialing Timeframe: 60-90 days if file is clean. will try to expedite

Submission Requirements

  • NCCPA - [REQUIRED]
  • Active OH license - [REQUIRED]
  • BLS, ACLS - [REQUIRED]
  • Training and Experience - [REQUIRED]
  • Providers must have 100% clean background to be considered, no exceptions - [REQUIRED]
  • A Self-Query NPDB report is mandatory at the time of presentation. The Self-Query NPDB report must be dated within 30 days of the presentation date. - [REQUIRED]
