Home Health Speech Therapist
Location: Raleigh/Durham, NC
The Home Health Speech Therapist will be responsible for providing medically prescribed speech therapy services to patients in their homes to improve or restore impaired function for patients under the direction of the physician. The therapist will work with patients who have communication and swallowing disorders, as well as cognitive and language impairments. The therapist will develop treatment plans in collaboration with the treating physician, monitor progress, and adjust treatment as needed. The therapist will also educate patients and their families on speech therapy techniques and exercises.
Personal Home Care of North Carolina (PHC) is a rapidly growing full-service home health agency looking for energetic and dedicated Speech Therapist for Raleigh/Durham, NC and surrounding areas.
ST Job Functions and Schedule will vary, but will generally include:
  • Evaluation and functional assessment of the patient and establishment of a plan of care.
  • Provide therapeutic techniques for the rehabilitation of patients.
  • Coordinate care, including care management and discharge planning as needed with patient/responsible party, physician, other disciplines and or external resources.
  • Documents accurately, objectively and timely in accordance with regulations and agency policy/procedures.
  • Requirements:
  • Master's degree in Speech-Language Pathology
  • Current state license to practice as a Speech-Language Pathologist
  • Minimum of 1 year of experience in home health or related setting
  • Experience working with patients with communication and swallowing disorders, cognitive and language impairments
  • Excellent communication and interpersonal skills
  • Ability to work independently and as part of a team
  • Valid driver's license and reliable transportation
