Key Responsibilities
  • Conduct thorough assessments to evaluate clients' social, emotional, and environmental challenges, ensuring their needs are accurately understood.
  • Develop individualized service plans that outline achievable goals, appropriate interventions, and continuous progress monitoring for each client.
  • Provide counseling services to individuals and families navigating lifeโ€™s difficulties, including emotional support and guidance.
  • Collaborate closely with clients to tailor service plans to their unique circumstances, linking them to essential community resources and support services.
  • Facilitate crisis intervention and perform risk assessments, ensuring client safety and offering appropriate safety planning measures.
  • Engage actively with community agencies to coordinate services, advocating for clients' needs while promoting effective resource utilization.
  • Participate in community outreach initiatives, increasing awareness of agency services and fostering connections within the community.
  • Attend case conferences, team meetings, and professional development training sessions, contributing to team knowledge and enhancing care strategies.
Patient Population / Client Focus

Your primary focus will be on individuals and families facing significant life challenges, such as terminal illnesses, mental health issues, and socio-economic difficulties. You will engage with clients who require advocacy and support to navigate complex healthcare systems and other vital resources, ensuring they receive comprehensive care tailored to their unique needs.

Qualifications
  • A Masterโ€™s degree in Social Work (MSW) from an accredited institution.
  • Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).
  • Strong understanding of mental health issues, trauma-informed care, and social justice principles.
  • Excellent communication abilities, cultural competence, and problem-solving skills required to work effectively with diverse client populations.
Preferred Experience
  • Prior experience in a social service agency or healthcare setting, particularly in hospice or palliative care environments.
  • Proficiency in case management and the use of electronic health record (EHR) systems for documentation and service delivery.
