ADULT PRIMARY CARE CLINIC SEEKS NP-C ADULT GERONTOLOGY

COMMUNITY

Chicago metropolitan area /city limits. Can live a variety of neighborhoods or suburbs. Culture, diversity, tons of events year round for the family.

OVERVIEW

Mission driven primary care group adding to its team of 4 providers to reduce hospital stays in a value based care model

ROLE
• Outpatient primary care responsibilities only; Monday-Friday 8am-5pm; No nights nor weekends
• New, state-of-the-art centers; welcoming patients with variety of acute or chronic conditions
• Support of scribe and medical assistant

• Collaborate closely with physicians and care teams to develop coordinated care plans

•  Patient Volume: 13-16 a day

• Sees patients on avg. every 3 months (more if complex cases)

• Equipment at clinic: Labs on site with a phlebotomist

• Cases can periodically require Suturing, Joint Injections, Skin Biopsies but mainly Preventive Screenings

• Salary Range: $120-$140k + (Based on Experience)
