Psychiatric Mental Health Nurse Practitioner (PMHNP), Full Time

Personalized Health Partners is currently recruiting for a Psychiatric Mental Health Nurse Practitioner (PMHNP) to join our team in the Indianapolis, IN area. Our employees enjoy competitive salaries and an outstanding, robust benefits package.

Position Summary:

The Psychiatric Mental Health Nurse Practitioner (PMHNP) will play a pivotal role in assessing, diagnosing, and treating residents in the long-term care facility setting. They will be responsible for developing personalized treatment plans for individuals with various behavioral health conditions including mood disorders, thought disorders, and substance use disorders. They will serve as leaders in a multidisciplinary team working to enhance the overall well-being of our residents.

Key Responsibilities:

  • Conduct thorough behavioral health assessments and evaluations for residents.
  • Develop and oversee individualized treatment plans in collaboration with residents, families, and interdisciplinary teams
  • Prescribe and manage appropriate medications and therapies tailored to the long-term care setting
  • Provide counseling and support to residents and their families
  • Monitor resident progress and adjust treatment plans as necessary
  • Educate residents and staff about behavioral health conditions and available resources within the long-term care setting

Minimum Required Education:

  • Master's or Doctorate Degree in Nursing with a specialization in Psychiatric nursing
  • Graduation from a state-certified advance practice program specializing in psychiatry/mental health

Minimum Required Experience:

  • At least 1-2 years of direct clinic care experience in behavioral health
  • Proficiency in working with both adult and geriatric populations.
  • Specialty NP experience in Behavioral Health
  • Strong interpersonal and communication skills
  • Previous experience in psychiatric nursing before transitioning to NP
  • Dedication to evidence-based practice and continuous professional development

Preferred Experience:

  • 3+ years of experience as a psychiatric provider
  • Previous experience in a Skilled Nursing Facility, though not mandatory

Licenses/Certifications:

  • Must have a certification by the American Nurses Credentialing Center (ANCC) in Psychiatric Mental Health (PMH) as a Psychiatric Mental Health Nurse Practitioner (PMHNP)
  • Must have an unrestricted license to practice as an advanced practice registered nurse (APRN) in the state of Ohio
  • Must have a valid DEA number and unrestricted DEA license in Ohio
  • Must have a state-issued Controlled Substance Registration (CSR) in Ohio
  • Must be registered with the state Prescription Monitoring Program (PMP) in Ohio where direct services will be provided
  • No suspension/exclusion/debarment from participation in federal health care programs (e.g. Medicare, Medicaid, SCHIP)
  • Maintain credentials and licensure with all major insurance companies and licensing boards with the assistance of Provident credentialing staff

Other Skills Required:

  • Experience in delivering high-quality, patient-centered care
  • Proficiency in documenting and navigating electronic health records (EHR) independently
  • Ability to maintain strict client confidentiality
  • Ability to serve as a representative of PHP, displaying courtesy, tact, consideration and discretion in all interactions with team members.
  • Proficient computer skills, including working knowledge of Microsoft Office Suite
  • Ability to work effectively on multidisciplinary team
  • Ability to read, analyze and interpret general and professional documents. Ability to write routine reports and correspondence. Ability to effectively present information in one on one and small group situations
  • Must possess the ability to make independent decisions when circumstances warrant such action
  • Ability to foster teamwork; train and direct the work of team members and colleagues
  • Excellent organizational skills, including the ability to work productively and make independent decisions
